Abstract
Penile inversion vaginoplasty remains the gold standard technique for feminizing genital surgery. This chapter provides a historical perspective on the development of this surgery and describes a variety of incisions for penile inversion vaginoplasty based on a patient’s anatomy. Creation of the neovaginal space is the most difficult portion of the operation, and a technique to safely do so is described by closely following the urethral catheter and developing a space through the prostate gland. Techniques to optimize labial and vulvar creation are also described.
