Abstract

Objectives: After completing this article, readers should be able to: 1. Define the transition process and the populations for whom it is most critical. 2. Identify barriers to transition for the practitioner, patient, and family and how to address them best. 3. Review existing legislation and local support services available to aid transition in the domains of education, vocation, and independent living for vulnerable populations. 4. Describe the central role played by physicians in the transition process.
