The treatment of anterior-superior instability with the pectoralis major muscle transfer

Abstract

Anterior-superior instability of the shoulder results from rotator cuff insufficiency, loss of the coracoacromial arch, and anterior deltoid compromise. The goal of treatment is primarily pain relief, as functional improvement is less predictable. Hemiarthroplasty in combination with a subcoracoid pectoralis major transfer has shown considerable promise for many of these patients. Proper transfer of the pectoralis muscle is critical for function and protection of nerves in close proximity. This article describes both the whole muscle and split transfer, rehabilitation, and known results for this procedure.
