Abstract
The International Continence Society (ICS) defines detrusor underactivity (DU) as “a contraction of reduced strength and/or duration, resulting in prolonged bladder emptying and/or failure to achieve complete bladder emptying within normal time span”. Currently there are no effective pharmacological therapies to increase the contractility of the bladder and so men are not infrequently offered outlet-reducing surgery as an alternative strategy to facilitate bladder emptying. Surgical outlet reduction is associated with highly variable outcomes in patients with DU and there is a lack of data on which urodynamic factors, if any, predict surgical outcome. In this chapter we analyze the role of outlet reduction surgery in both men and women with underlying DU and the outcomes from the available studies in the literature.
