N1 - Funding Information: Our clinical practice guidelines committee has been extremely productive under the leadership of Dr. Suhny Abbara. We have published a landmark statement to help guide reporting and management recommendations following CTA, entitled the CAD-RADS statement (Coronary Artery Disease Reporting and Data System), co-sponsored by the American College of Radiology and endorsed by the American College of Cardiology (lead author: Dr. Ricardo Cury). A similar document is underway supporting recommendations for coronary artery calcium (CAC) scoring (lead author: Harvey Hecht). Additional guidelines have been published on the evidence and indications for CAC of a noncontrast, noncardiac chest CT scan (lead author: Harvey Hecht) and the performance and acquisition of coronary CT angiography. Of all the clinical guidance documents, one that stands out relevant to CTA is from the United Kingdom's National Institute of Health and Care Excellence (NICE) (CG-95). This guidance document on new onset chest pain recommends CTA as the index procedure, regardless of the patient's pretest risk. This is a landmark guidance document that revolutionizes the evaluation strategy for stable chest pain and places CTA as the index procedure to optimally guide medical therapy and selective use of functional testing or invasive angiography. The NICE estimates that the National Health Service will realize considerable cost savings by placing CTA as the core procedure for the evaluation of stable chest pain.
