Abstract

Bilateral orchiectomy is an important surgery for transwomen and nonbinary people. Not only does removal of both testicles help conform to gender identity, but it also decreases testosterone production, removing the need to take antiandrogen medications. Bilateral orchiectomy is a safe outpatient surgery that can be performed by most surgeons without much additional training. This chapter highlights several key surgical points and differences between a simple orchiectomy for castration and orchiectomy of an atrophied or dead testicle.
