Abstract
The case of a 2 1/2 year old boy is reported in which a separation of the dual effects ofangiotensin II were demonstrated: elevation of systemic blood pressure and stimulation of aldosterone secretion. The patient had hypokalemia, hyposthenuria, and growth retardation. The excretion of aldosterone was elevated; the serum concentration of renin was increased, and elevations in blood pressure in response to both angiotensin and renin infusions were inadequate. Hyperplasia of the juxtaglomerular apparatus was seen on renal biopsy. Studies obtained on the parents and siblings of this patient may be compatible with a genetic basis for this disease.
