Abstract

To the Editor: We are not surprised that Frigoletto et al. report good outcomes without intrauterine transfusion in pregnancies with rhesus sensitization, described in their recent article (Aug. 14 issue).1 Except for one value of 0.395, the amniotic-fluid optical-density difference at 450 nm (ΔOD 450) in all cases was between 0.162 and 0.26. Few would advocate routine intrauterine transfusions for fetuses without hydrops whose amniotic fluid had optical densities in this range. Even an Rh-negative infant can occasionally have values of 0.2 to 0.25 at 22 to 24 weeks.2 Serial amniocenteses, ultrasound examinations, and nonstress tests are needed to guide…
