The efficacy and reliability of these referrals depends on mutually beneficial, long-term relationships with trusted community-based organizations (CBOs). We applied for and received grant funding from the United Hospital Fund (UHF) and the New York Community Trust (NYCT) to promote clinical-community partnerships. Under these grants, we had two key CBO partners in East Harlem, the New York Common Pantry (NYCP), and LSA Family Health Service (formerly known as Little Sisters of the Assumption Family Health Service), with whom we met quarterly to analyze referral patterns, provide feedback on referrals made, and streamline the referral process.
