Hepatocellular carcinoma: Patients with increasing alpha-fetoprotein but no mass on ultrasound

Abstract

A 63-year-old man with hepatitis C virus-induced cirrhosis is referred to the clinic for evaluation of elevation in the serum level of alpha-fetoprotein (AFP) to 400 ng/mL. He is active at work and asymptomatic, and has compensated liver cirrhosis and well-preserved liver function. Ultrasound reveals hepatomegaly without any liver mass, preserved portal blood flow, and splenomegaly. Read the differential diagnosis and management on page 29.
