Abstract
Despite significant advances in the medical field, pediatric heart failure continues to have a significant health burden. ACTION (Advanced Cardiac Therapies Improving Outcomes Network) was formed to bring the benefits of a learning health system to the problems of pediatric heart failure and VAD care. The ACTION learning collaborative network has expanded its focus to incorporate quality improvement initiatives aimed at improving outcomes in pediatric heart failure. The multiple initiatives include a collaboration with the Pediatric Health Information System (PHIS) database, timely referral for heart failure management in Fontan patients, formation of a waitlist outcomes committee, and a heart failure management committee with numerous parent and provider education initiatives.
