Dobutamine stress echocardiography: Does it predict response to beta-blockers in patients with heart failure?

Abstract

In patients with chronic heart failure (CHF), β-blockers have been shown to improve symptoms, exercise tolerance, functional capacity, quality of life, left ventricular ejection fraction (LVEF), and survival and reduce sudden cardiac death. However, the effect of β-blockers in patients with CHF has also been shown to be heterogeneous, with improved LVEF in some and worsening or no response in others. This review summarizes studies evaluating the role of inotropic contractile reserve (ICR) with dobutamine stress echocardiography in predicting responders to β-blocker therapy among patients with heart failure. It also reviews the rationale for the beneficial effects of ICR in this cohort.
