Diagnostic difficulties in peripheral cholangiocarcinoma

Abstract

Intrahepatic cholangiocarcinoma is a rare disease. Cholangiocarcinoma is the malignant tumor arising from the bile duct epithelium; it accounts for less than one fifth of primary liver cancers. In this article the authors present five cases of cholangiocarcinoma (consisting of four resected and one autopsied cases). All cases were analyzed in terms of clinical characteristics such as: gender, age, initial symptoms, reintervention for recurrence, hepatitis viral markers and cirrhosis. Pathologic characteristics were also analyzed: tumor size, the site of tumor in the liver, macroscopic type, microscopic type, venous invasion, lymphatic permeation, intrahepatic metastasis and lymph node metastasis. Distinction between primary and secondary malignant neoplasms of the liver (particularly cholangiocarcinoma versus metastatic adenocarcinoma) is often difficult on the basis of light microscopy alone, even using immunohistochemical analysis.
