N2 - Type 2 Diabetes Mellitus (T2DM) is a prevalent and costly condition, particularly affecting racial/ethnic minorities and low-income populations in the United States. Comprehensive Medication Management (CMM) programs, where clinical pharmacists co-manage patients alongside primary care physicians, offer a team-based approach that has been shown to improve diabetes care. However, managing CMM programs across multiple sites presents operational challenges that can be addressed through digital tools. This “how-to” guide provides an overview of developing and implementing a CMM Dashboard to enhance program management and patient outcomes in a large municipal health care system. Key components include tracking patient demographics, clinical outcomes (e.g., hemoglobin A1c, blood pressure), and operational metrics across 21 sites. Practical applications of the dashboard are outlined with a focus on real-time data analysis, care gap identification, and optimizing resource utilization. By consolidating comprehensive program data, the CMM Dashboard supports clinical decision-making, informs leadership on program performance, and enables targeted interventions to improve health outcomes for underserved populations.
