Abstract

Coronary artery calcium (CAC), a marker of subclinical atherosclerosis, has emerged as the most robust predictor of adverse cardiovascular events in the asymptomatic population; it is superior to all risk factor-based scores including the Pooled Cohort Equation in the 2013 ACC/AHA Cholesterol Guidelines. It is most useful in reclassifying patients in the intermediate risk group to lower or higher risk categories and has been incorporated into guidelines as part of the decision-making process in selected circumstances. Progression of CAC on serial scanning is indicative of poorer outcomes.
