Abstract
Rosenberg Center staff sought to improve the questionnaire's design in order to produce evaluations that would specify unit locations and services. After examining the unit structure in detail, Rosenberg Center staff chose unit supervisors as the central agents responsible for implementation and follow-up, and unit clerks would be responsible for distributing the forms to patients. Statistical reports can be stored cumulatively for analysis of trends on a quarterly and yearly basis.
