Abstract

Brain metastases are the most frequent brain tumors and are a medical challenge. Tumors have a different ability to metastasize to the brain and must have the ability to penetrate the blood-brain barrier, interact with resident cells, and survive. The clinic is varied depending on the site affected, as well as the associated risks of seizure.Genomically it has been increasingly documented that brain metastases change the expression of their mutations. Thus, the choice of treatment is complex, considering the primary, the number of metastases and affected sites. Radiation therapy has long been the choice for patients who are not candidates for surgery, and advances are expected to reduce especially cognitive toxicity. The genomic knowledge of metastasis to the brain and the presence of target therapies and modified immunotherapies penetrating the blood-brain barrier have been key, it is hoped in the future to have more studies of combinations of radiotherapy treatments with target and immunotherapy therapies.
