N2 - To the Editor: We do not agree with the discussion and conclusion presented by Dr. Sheldon M. Wolff in the Case Record 18–1980, which appeared in the May 8 issue. In addition, we think that the anatomical diagnosis is not the appropriate one. The history of wheezing with fever and lung infiltrates, which disappeared on administration of low doses of steroids, is more suggestive of the Churg–Strauss syndrome1,2 than of the hypereosinophilic syndrome. The acuteness of the effects on the heart is not a common feature of the latter syndrome.3 4 5 This feature, together with the patient's Q waves, pericardial friction.
