Abstract

In ischemic stroke aspirin has shown its prophylactic efficacy even in low doses. Heparin treatment prevents early recurrence of embolic stroke and has only a mild increased risk of intracerebral hemorrhage. In cerebral sinus and vein thromboses heparin is regarded as safe and efficacious therapy even in cases with preexisting hemorrhage. Local intraarterial thrombolysis has shown its efficacy in the vertebrobasilar territory and demonstrates promising results for the carotid- and middle-cerebral artery territory. Ongoing studies will prove the efficacy and will evaluate the potential risk of systemic application of clot-specific fibrinolytic agents (e.g. rt-PA).
