Assessing Goals of Care: A Case-Based Discussion

Abstract

In the setting of serious illness, personal goals and values must guide complex treatment decisions. Without an understanding of these goals, development of a person-centered treatment plan is not possible. Yet eliciting and clarifying these goals require significant skill on the part of the clinician. In this chapter, we use a patient case to illustrate the steps and skills needed to elicit goals of care and provide examples of helpful language. In addition, we offer guidance for several common challenges: using a medical interpreter, working with a surrogate decision-maker, and exploring the role of religion and spirituality.
