Adjuvant therapy induced vasculopathy following macular hole surgery

Abstract

Purpose. To report a previously undescribed complication of adjuvant therapy for treating macular holes. Method. A 58 year old female with a Stage III macular hole underwent pars plana vitrectomy, removal of the posterior hyaloid and application of autologous plasma and tissue thrombin without complication. Thirteen percent C3F8 was injected post-operatively to achieve an approximate 80% fill on day one. Results. Upon clearance of the C3F8 gas bubble, perifoveal intraretinal hemorrhage was discovered. Fluorescein angiography revealed a vasculopathic process characterized by capillary non-perfusion and intraretinal hemorrhage. The findings cleared within six months. Conclusions. Adjuvant therapy in macular hole surgery may cause perifoveal vascular decompensation.
