Abstract
Anticoagulants are effective for preventing and treating thromboembolic disorders. However, these medications are associated with risk of bleeding complications that can lead to hospitalizations and death. Community pharmacists are well positioned to improve outcomes for patients on anticoagulants by identifying and addressing medication-related problems, as well as providing patient education.
