Targeted health dialogues in primary care in southern Sweden: Lifestyle screening and molecular profiling with long-term follow-up in 40-, 50- and 60-year-old persons

Project Details

Description

Purpose and aimsThe overall purpose is to provide new knowledge for more effective personalized interventions for cardiovascular health. The specific aims are: to evaluate the effect of targeted health dialogues on cardiovascular disease (CVD) and CVD risk factors; to examine confounding and modifying effects by sex, sociodemographic status, familial factors and comorbidities; and to establish a biobank in order to identify novel molecular CVD risk markers. Project organization, time plan and methodsThe Skåne County Council has initiated targeted health dialogues of all 40-, 50- and 60-year-old persons in primary care. The collected data in this prevention program will be aggregated in a health curve and used by specially trained health dialogue leaders. Our cross-disciplinary research group at the Center for Primary Health Care Research will integrate a unique longitudinal research project within the program where we will link questionnaire data, cholesterol, blood glucose, BMI, waist-hip ratio and blood pressure to clinical diagnoses and population data as well as genomics, proteomics and metabolomics. All data will be collected continuously (as in our recent pilot study) and findings will be presented during year 2 and 3.ImportanceThis large-scale study will optimize lifestyle interventions for underserved population groups. We will also identify novel biomarkers in order to design more effective personalized CVD prevention in primary care settings.
